About You

You are looking for a rewarding career and are a dedicated Support Coordinator who is passionate about helping people who have a disability to live their best life! You will collaborate creatively and resourcefully with clients to maximise chances to satisfy their support requirements in order to best achieve their goals. Prior experience as a Support Coordinator is not required, however it is advantageous if you have transferrable skills or worked in a related field. Previous experience in age care, case management, youth justice, employment services or child safety is a great base to start as a Support Coordinator.

 

You will have:

  • Previous experience in Support Coordination or transferrable skills
  • Experience in Psychosocial Disability (advantage)
  • A basic understanding of the NDIS and Support Coordination framework
  • High level of interpersonal skills
  • Ability to network and work collaboratively with internal and external stakeholders
  • Ability to thrive in a fast-paced environment with continuous improvement practices

 

About the Opportunity
This is a full-time role based in our Rockhampton office. You will be joining an experienced and knowledgeable team that have a passion for working with clients, coordinating their supports under NDIS. You will work closely with your clients to effectively utilise the supports funded in their NDIS plan, connect them with suitable service providers and allied health professionals, and monitor the progress towards achieving their plan goals.

 

Tasks & Responsibilities:

  • Work with clients and their families to assist them to meet their goals and get the best out of their NDIS plan
  • Assist client to build their capacity around the understanding of their plan and its implementation
  • Develop strong plan implementation practices for the duration of the NDIS plan
  • Establish and maintain strong working relationships with service providers and allied healthcare professionals
  • Provide plan review reports to the NDIA clearly articulating clients needs throughout their NDIS plan and beyond
  • Provide effective, responsible and transparent services to the clients

Mandatory Requirements:

  • Driver’s Licence
  • Blue Card
  • NDIS Worker Screening Clearance
  • Up to date COVID-19 Vaccinations
  • NDIS Worker Orientation certificate
