About the role
As a Full-Time Registered Nurse Case Manager you will be responsible for the planning and implementation of Community Models of Care for customers with complex clinical needs.

In this role, you will provide support to clients in Rockhampton and surrounds. Supported by an experienced, professional team your duties will include:
  • Leading case management for high level support programs, i.e. HomeCare Package Programs Level 1 to 4 and other funding programs such as CHSP
  • Upholding clinical governance and managing the quality management framework within Home Care services to ensure care standards are met
  • Coordinating the planning and implementation of a Model of Care
  • Providing direct and indirect clinical care to customers within scope of practice and ensure clinical stocks are maintained
  • Ensuring customer records, incidents and complaints are documented and maintained correctly and participate in any investigations as required
You will also be a strong leader who provides guidance, support, and direction to staff in nursing care and service delivery on shift as required.
About You

  • Understanding and experience in the delivery of health services and/or community-based services as well as complex case management
  • Proven ability to lead a team and provide positive stakeholder outcomes
  • Knowledge of the Aged Care sector as well as HomeCare Packages is highly desirable
  • Switch on and forward-thinking persona with strong abilities in Microsoft Packages
  • Fast Learner and abilities to pick up new systems/software’s quickly
  • Current drivers’ licence in the state or territory where your employment is based, as well as a comprehensive insured, registered motor vehicle
  • 2021 Flu Vaccinations (or willing to obtain)
  • Double covid-19 vaccination evidence 
  • Bachelor of Nursing or other qualifications and/or training deemed equivalent by AHPRA leading to registration as a Registered Nurse (essential)
  • Current nursing registration with AHPRA as a RN
  • Current Senior First Aid Certificate and CPR
