Position Summary
We’re seeking applications from experienced community services Care Coordinators to lead and support our personal care team and social wellness initiatives.

This newly created full time position in our Central Queensland community services leadership team is a great opportunity to secure a permanent contract in a client focused aged care provider actively setting new standards in how community aged care is delivered.

As a Social Wellness Care Coordinator working for Blue Care, you will support and lead a team of social wellness and care professionals working in the community. You’ll be driven to provide outstanding care to clients in your region, advocating individualised care that enhances and promotes their quality of life. In collaboration with your service manager you’ll empower your team through learning, support and leadership in order to advance the level of service to our clients.

Working closely with your Community Services Manager, you’ll ensure quality and compliance governance are achieved through robust processes that meet internal and external auditing standards.

You’ll ensure the sustainability and growth of the service through your diligence with community funding, care planning and community engagement. You’ll plan and implement social wellness initiatives centred on quality improvements, driving positive changes within your services.

What you’ll need:
As an experienced community services professional, you’ll hold considerable knowledge of community care with practical experience leading personal care teams and social wellness initiatives within an aged care & community setting. You’ll have a solid understanding of the industries quality standards, frameworks, funding and how these have shaped care delivery for services. You'll have:
  • Excellent written and verbal communication skills
  • Proven leadership, coaching and development skills
  • Sound experience in management of budgets & financials
  • Ability to routinely problem-solve complex scenarios and conflicting priorities
  • Proven experience in planning, implementing and monitoring social wellness initiatives.

Selection Criteria
